Vision assessment questionnaires are very important to guide testing during an assessment and also serve as a useful benchmark of symptoms and difficulties before versus after a vision treatment. The more complete the information the more we are able to target the assessment to your needs.

Questionnaires must be completed at least 2 weeks in advance of your appointment date.
